The GLP-1 boom's binding constraint was never the molecule - it was sterile fill-finish: getting drug into device at scale. Inside that constraint sits a quieter one: the elastomer stoppers and plungers that touch the drug, a market West Pharmaceutical dominates from a town in Pennsylvania.

1The constraint was the vial, not the molecule

Semaglutide and tirzepatide spent 2023-25 on the FDA shortage list not because synthesis failed but because sterile fill-finish - filling cartridges and syringes, assembling auto-injector pens, releasing them under aseptic conditions - could not scale as fast as demand. Lilly and Novo have poured tens of billions into captive capacity, and Novo's parent bought a CDMO outright: Novo Holdings' $16.5 billion acquisition of Catalent, closed December 2024, was a fill-finish deal wearing an M&A press release.

Every filled unit, whoever fills it, contains the same class of component: an elastomeric stopper or plunger in contact with the drug.

2West's quiet dominance

West Pharmaceutical Services (WST), headquartered in Exton, Pennsylvania, is the dominant maker of those components - industry estimates commonly put its share of injectable elastomers around 70%, with Datwyler (Swiss, listed in Zurich) the main alternative and premium lines like NovaPure specified by name in biologic filings. The share is an estimate and we flag it below; the mechanism is not: components that touch a biologic are named in the regulatory file, and changing them means stability studies and refiling.

That is the same process-embedded lock-in as ABF in substrates or chucks in etch chambers - the moat is everyone else's paperwork.

3How the exposure actually behaves

WST's GLP-1 leverage is volumetric: pens consume plungers and stoppers per unit, so prescription trends flow through with a lag through customer inventory - which is also the risk, as 2023-24 showed when post-pandemic destocking hit the stock hard even while GLP-1 volumes grew. The honest framing: WST is a royalty on injectable volume generally, GLP-1 included, minus inventory noise.

Adjacent listed layers: glass cartridges and devices (Gerresheimer, Schott Pharma, Stevanato in Europe), bioprocessing upstream (Thermo Fisher, Danaher). The drugmakers themselves - LLY, NVO - are the demand signal.

4Watchpoints

Track: FDA shortage-list status for the incretin franchises (both cleared it as capacity landed - re-entry would signal the gate is binding again), WST's high-value product mix and destocking commentary, and capacity announcements measured in filling lines, not dollars. The falsifier for the component thesis would be a major biologic switching elastomer supplier mid-life - watch for it; we could not find a precedent, which is rather the point.
